identification of mental disorder

respected sir, I am 19yr old boy request you to name my mental disorder (from which mental disorder I am suffering from) I deffinitely go to doctor but just for my confirmation, I want to know the name of my mental disorder, my symptoms are {NOT able to concentrate, not at all confidence, extreme nervousness, extreme shyness, unnecessary worrying I cant stop my mind from worrying, talkativeness, feeling like going mad, fear to face new situation, not able to walk in particular manner, sometimes mind does not work, lack of happiness, unable to make any kind of decision (any), racing thoughts, afraid to talk to new people, afraid to talk on phone, cant able to talk with brother and sister properly- feeling like anger with them, very disturbed sleep, SINCE CHILDHOOD FACING THIS PROBLEM, ME AND MY BROTHER WHO IS TWO YEAR YOUNGER THAN ME.
